Dear Parents/Guardians:

It is with great pleasure that I welcome you and your family to Harvard Public Schools!

Following this letter you will find a various links to registration forms required for enrolling your child(ren) in our school system. Kindly complete the various forms and return them to this office at 39 Massachusetts Avenue, Harvard, MA, 01451. If you have any questions regarding this process, please contact Jackie Smith, Registrar, at 978-456-4140, who will be happy to assist you.

Please note that the following information is required for first time registrations:

• Proof of residence (i.e. Purchase and Sales Agreement, copy of lease or electric bill)
• Copy of Birth Certificate
• Proof of immunization
• Copy of last complete physical examination

Once the information is received and processed, the Guidance Office will be notified of your child(ren)'s registration. I would encourage you to schedule an appointment with the appropriate Guidance Department. Their telephone numbers are as follows:
