    All Children Entering Kindergarten are required to have the following
    immunizations:
                  *5 doses of DTaP vaccine
                  *4 doses of polio vaccine
                  *2 doses of MMR vaccine
                  *3 doses of hepatitis B vaccine
                  *2 doses of varicella vaccine (or certified history of
    chicken pox disease)

    *While not required, annual influenza vaccination is recommended for
    all school- aged children.
